Privacy Policy

This policy was made in light of the requirements set out by the General Data Protection Regulations (GDPR) 2018 in order to explain our data processing practices. It explains the basis on which we will process any personal data we collect from you, or that you provide to us. This policy will include:

Who do we collect data from?

We collect data from all clients who are actual service users or potential service users who show an interest in our service.

What information do we collect from you?

We ask all existing or potential clients to complete a client information form.

This includes:

  • Full Name
  • Address
  • Date Of birth
  • Contact Telephone
  • Numbers
  • Email Address
  • Profession
  • Previous Treatments

 

At each attendance we complete a treatment sheet with your personal details completed again and an explanation of the treatment you have received on that day.

We will take before and after treatment photographs of you to ensure that any treatment has been successful and have given the client the outcomes they were expecting.

We do not collect or store any payment or bank details from clients.

How do we store your information?

When you complete your treatment / consent form, it is stored securely in a locked filing cabinet which is only accessed by the owner of the company (Mrs Annie Coles).

How will we use this information about you?

The information we collect about you is collected to ensure that you receive the best service from us.

Particular information is required to complete a prescription for certain treatments. This is a legal requirement and a prescription cannot be written without it.

We complete treatment information to keep a record of all treatment you have received from us, any side effects or adverse effects from the treatment.

If you stop using our services, this information will be stored for a period of 7 years and then discarded in a confidential manner.

We will use your email address and/or mobile phone number to send you information about any discounts, competitions or special offers we are offering (unless you explicitly opt out from receiving these communications – just let us know).

We will also send you a reminder of any appointments or queries about upcoming appointment dates or times.

We will use anonymised photographs on our advertising with your specific consent.

Who has access to the information we hold about you?

We do not send your details to any other person without your explicit consent.

This would only be for the purposes of a second opinion or a referral to another colleague.

We do not send your details to anyone for marketing purposes.

Access to your information

You have the right to request a copy of the information that we hold about you.

We keep our privacy policy under regular review and we will keep an updated policy in the clinic that you may request a copy of at any time. This privacy policy was created on 11th October 2019, and will regularly updated in-line with GDPR.
